Group Leaders: to reset a user’s password, perform the following steps

Log into GIDER.BEAM.CARE and press Group Control from the menu area:

Find the user or other Group Leader whose password you would like to change, and press the Manage User link.

Scroll to the bottom of the Manage User page and enter a new password into the Change Password Field

If a Group Leader cannot access their account, and they are the sole Group Leader in their group, they will need to contact us via email at support@createabilityinc.com or telephone at 3177286670.

Flourish is a person-centered planning app you can download from the Google play store or the Apple app store

You can create a basic person-centered plan for a single person on the Flourish app, or you can build a more detailed plan with objectives & scheduled tasks for multiple clients via the CreateAbility cloud at GIRDER.BEAM.CARE

This article will focus on creating detailed Flourish plans for multiple clients on the CreateAbiliy cloud at GIRDER.BEAM.CARE

Requirements

You must belong to an organization with access to the CreateAbility Cloud and have clients assigned to your username.

To Create a Plan

Log into GIRDER.BEAM.CARE and select the client you want to create a Flourish plan for.

If you don’t have clients assigned to you, contact your administrator or contact us. If you’re an administrator, create clients on the Group Controls page and then assign the clients to users with the Manage User links on the Group Controls page.

Press Create Plan.

Follow the prompts to write a plan, and upload images that will help the person you serve to engage with their plan.

If you’re unsure what to write, there are clarifying instructions and example plan statements in the information buttons next to each item.

Developing Goals

When you’ve completed the forms on the first onboarding page, press the Continue to Developing Goals button at the bottom of the page

This is where you can define goals, and upload images that will help to remind, motivate and engage the supported person to pursue those goals.

Follow the prompts to develop goals, and use the information button to get more context about goal setting.

Short-term goals support date ranges that care professionals can input and reference to ensure a goal isn’t addressed or completed in the appropriate time frame.

Creating Objectives and Objective Tasks for short-term goals and goals recommended by others

For each short-term goal and goal recommended by others, you can create objectives and tasks. Objectives are meaningful touchpoints to completing a goal.

To create an objective for a goal, press the Add an Objective button beneath the goal you’d like the objective to fall under. Then fill out the objective field.

You can add as many objectives to a goal as you’d like

Objective Tasks

Objective Tasks are tasks the supported person can do to complete an objective. The task will appear in the supported person’s today view for the date range you define.

When you’ve completed developing goals, press Finish at the bottom of the page.

Congratulations on creating a Flourish Plan!

Now you can copy the Flourish User ID into the Flourish app, and hand off the device to the person you support!

Logging in with a Flourish ID in the Flourish app

Download and open the Flourish app on a tablet or smart device of the person you support. Press Login/Settings

After logging into GIRDER.BEAM.CARE, go to the menu and press Flourish Manager.

Copy the text from the Flourish Login ID table

Go to the Flourish and go to settings in the menu by pressing the gear icon for 5 seconds. Copy the Flourish Login ID into the Flourish User ID field.

Troubleshooting

Uploading photos and images

To upload photos to a person’s plan in the GIRDER.BEAM.CARE, select an image to upload by pressing Choose File. Then follow your system’s prompts to select a picture file and confirm, when you return to the Flourish onboarding page, press Upload

Flourish enables you to create plans from the cloud or on the Flourish app itself at any point in the onboarding journey.

An important item to note is that there needs to be fully completed text information present in a particular section to switch from cloud onboarding to the mobile app onboarding

Changing a Flourish user’s name

You can change a Flourish user’s name by going to their System Configuration in the CreateAbility cloud at GIRDER.BEAM.CARE and changing the Client Name field

As a Flourish administrator for your BEAM group, you have the ability to create user roles with different privileges and abilities for fine control over how people in your organization can use the Flourish cloud.

Creating Roles

Because no company or organization is built exactly the same way, Flourish gives you the ability to create custom roles for your team, with custom privileges for viewing and editing Flourish users’ plan.

To create new user roles for the support professionals in your organization, go to GIRDER.BEAM.CARE and log in with your administrative credentials.

If you don’t have an administrative account and would like one, contact us.

Go to Create Roles in the Flourish Menu

Enter the Role Label for the role you’re creating. For example, if the role is for a support professional, you can type Support Proffesional into the field.

Below the Role Label are the Flourish Page Privileges:

These are the privileges that the new role you’re creating will have. Scroll through and press the buttons for each corresponding privilege you would like the role to have.

For instance, if you want Support Professionals to be able to only view plan pages, you would press the View Only radio button for each page category.

When each page has the appropriate priveleges for the role you’re creating, press the Add Role button near the bottom of the page. The new role will appear in the Existing Roles

Getting Flourish

Download the Flourish app from the Apple App Store or Google Play Store, or contact for a device loaded with the Flourish app.

Using Flourish for the First Time

Open Flourish on your device

Press the “Go to Flourish” button

If you are using the cloud-enabled version of Flourish for care professionals, press login and enter the Flourish ID of the person you’re supporting.

Onboarding

Onboarding is the process of getting the person you serve ready to use the Flourish app.

Begin onboarding the Flourish app by filling out the forms:

My Cover Page

This page is for communicating the person’s personality in terms of interest and experiences.

Here, you can place a high-quality picture or selfie of the person that shows them in a setting of their choice. It’s best not to use a photo that looks like a “mugshot.”

Name:
Enter the person’s preferred name here.

The Importance of Gratitude:
Sharing experiences of gratitude helps introduce the person to others in a very positive way.

A spirit of gratitude leads to positive emotions.

A spirit of gratitude creates positive relationships.

Gratitude makes life more satisfying and allows people to share and remember positive thoughts and experiences.

Here are some examples of positive statements of gratitude:

“I am grateful for my Mom, Dad, and Ellen, my sister.”

“I am grateful for my job.”

“I am grateful for my friends.”

I Am Proud Of

On this panel, feel free to share things the person is proud of owning, things they have recently accomplished or tried as well as any new changes in their life.

It’s OK to boast a little bit.

Here are some example statements:

“I have a new job.”

“I am going to a new church.”

“I joined the Model Railroad Club.”

Keys to Coaching Me

This section is for sharing information with the people who are working closely with the person to provide the supports that they may need or desire.

Share what works best for the person when they’re having a rough day — words of encouragement, knowing why something should be done, helping change their focus, how best to help when they’re angry or upset, etc.

Also, share different ways the person calms themself down if they get upset — going to their room, going for a walk or listening to music are some examples. If certain words or topics are “hot buttons” for the person, share that as well.

My Wellness

This page is for communicating the person’s personality in terms of interest and experiences.

This is for sharing things or items that engage the person’s attention, including things they like to do.

This section can often be lengthy, which is a good thing.

As a result, bullet points can be used.
This list may include the person’s hobbies, church attendance, sports teams, physical exercise, such as “riding my bike,” certain TV programs, and favorite restaurants, among many possibilities.

Here are some example statements:

“Building Lego kits.”

“Going out to eat.”

“Riding my bicycle.”

It will help to do this with the person you are creating a plan for

Developing Goals

Flourish lets you set goals in four specific areas:
Near-term—goals that you believe you can accomplish in the next year

Mid-term—Goals you believe may take two to four years to accomplish

Long-term—Goals you feel may take more than 4 years to accomplish

These time periods are the dates you are setting for the person to accomplish their goals. This isn’t concrete but will be useful to the person as a planning guide.

The “Goals Suggested by Others” page does not follow a timeline but rather includes goals suggested by other people in the life of the person served.

It helps if the person can be specific about their goals to tell if they are making progress towards them or have accomplished them. A goal such as ‘I’d like to lift weights’ could include information about frequency (I’d like to lift weights twice a week) if this is important to the person.

If setting personal goals is new to the person or if they find it difficult to work on or follow up on your goals, it may be best to begin with fewer goals rather than developing too many.

Near-Term Goals

In this first goal area, the time frame to consider is short-term—from the day we write them down to up to one year. Hopefully, the person will accomplish many of their Near-Term goals sooner than a year. As a result, the person may be developing more near-term goals than those that take longer.

Remember, you may need to break down some goals into steps. Think about a carpenter—he must first build walls before he can put on the roof.

Some examples:

‘I would like to spend more time with Bob and Jim playing video games—at least one day every weekend.’

‘I want to play a musical instrument, maybe a guitar, but I’m not sure.’

‘It would be great if I could visit my grandparents in Indiana.’

‘I would like to have some money so I could buy my sister a Christmas present.’

Mid-Term Goals

This goal time period is from about 2 years to 4 years. This area of your ‘My Plan to Flourish’ may be the most difficult to set in terms of selecting an exact time. There are many reasons why—this time frame may involve learning a new skill, changing your mind, saving money or having unexpected things happen.
Again, it is quite likely that the goals identified in this section will need to be broken down into steps and that you may need some assistance to accomplish the goal.

Some examples:

‘I would like to have a steady girlfriend that I can go on dates with.’

‘I could do more things if | learned how to take the bus downtown or to Wal-Mart.’

Long-Term Goals

Think about this time frame as requiring more than four years. These are the hopes and dreams the person has for their life. Ideas may vary over time but could involve friends and relationships, work or career goals, where they live, taking classes, learning a new hobby, completing a training program, or even planning for retirement.

Most of us change or modify our dreams for the future because we have gained new information, perhaps have developed a physical limitation or experience a lack of sufficient resources.

It is important to have long-term goals because it allows us to view the future positively and think not only about the present, but how we would like things years from now.

Some examples:

‘I would like to have a job as a car mechanic in the future.’

‘It would be nice if I could live someplace where it was warm in the winter, maybe with a close friend.’

‘I would like to go to NASCAR races whenever I want to.’

Goals Recommended By Others

In the last goal area of Flourish, we identify the goals recommended by others. Such goal recommendations may come from our friends, parents, support staff and, many times, from our doctors.

Some examples:
‘After talking with my doctor, I agree that I should go for a walk every day to help lose some weight.’

‘I know sometimes | get too angry. I will work on following the anger management program that | have and go a month without breaking any objects.’

‘I am willing to go to the dentist if I can meet her first and see if I like her.’

Editing Flourish Pages

Open the menu in Flourish and press the gear icon for 5 seconds, then pick what part of the plan you’d like to edit and press the edit button.

This article is for administrators that want to learn how to create GIRDER.BEAM.CARE accounts for their staff, and systems for clients, respectively. This article also details how to assign a client’s system to a staff member’s user account.

Creating accounts for staff (Users)

Creating systems for Clients requires administrative privileges for your Girder.beam.care group. If you don’t see the Group Controls menu item when you log into Girder.beam.care, contact CreateAbilty Support at support@createabilityinc.com

If you can see the Group Controls menu item in Girder.beam.care, press it.

To create a new user account for a staff member, press Add New User

Choose a username, email, and secure password for the user.

Its important you have access to the associated e-mail for retrieving forgotten passwords or receiving alerts.

Creating accounts for clients

To create a new system for a client to use with their CreateAbility apps, press Add New Client.

An unnamed client will populate at the bottom of the clients’ list as [UNNAMED]. Press on the client.

You can name the system after your client’s name or any other naming convention you prefer, and press Send.

Assigning Clients to Users

Select the staff member you would like to assign a client to in the Users table of the Group Controls and press Manage User next to the user’s name.

A new page will open showing what clients are associated with that user.

To add a client’s system, select their name from the Add New Client dropdown menu beneath the systems table and press Submit.

Finding the Client ID for clients’ apps.

CreateAbility’s HIPAA compliant apps utilize Client ID to connect supported persons’ data to our secure cloud. If one of our apps asks for a Client ID or Agency Code, you can find them in the group controls

You can find the code next to a client’s name in the Group Controls page. If you don’t have administrative privileges, you can find the code of the person you serve in the browser URL when you are in the client’s system on girder.beam.care.

The Client ID is used to connect clients to certain CreateAbility Apps, Namely:

Flourish

OnTracker

MyBrain & MyBrain Now

403 Errors

If you’re receiving a 403 error while trying to navigate to a client’s system:

  1. You do not have access to the client’s system and need to contact the administrator of your GIRDER.BEAM.CARE cloud group.
  2. If you are an administrator or group leader, You are trying to access the system from a Manage User page of a user other than you.

To get to the desired client’s system, navigate to Group Controls – then go to your username and press Manage User.

Add the desired client to your account with the Add New Client widget and press Submit.

Finally, press the group logo in the upper left side of the page.

You should be able to access that client from your System View.

If you cannot access the system after performing the steps, contact CreateAbilty support so we can remediate the error for you!

Restoring Plans in Flourish

With Flourish and it’s cloud component BEAM.CARE, you can back-up the person-centered plans you create so that you can access and edit them from anywhere you have a mobile device and the Flourish app.

To restore or back-up a plan:

  • Open Flourish on the device of the person you’re supporting
  • Press the app settings link at the bottom of the splash screen with the Flourish logo.
  • In the field labeled User Code, enter the unique code issued to you CreateAbility Inc. or your organization.
  • Next, Press Restore.
  • And finally, press Save. Flourish will restart with the restored plan data.
  • If you want to perform a back-up or restore from the plan section of Flourish, you can access the settings page by pressing and holding the header in any of the plan sections and pressing APP SETTINGS in the appearing popover widget at the bottom of the page.

You can also edit your plan panels by pressing the EDIT button directly above app settings.

Remotely editing a Flourish Person-Centered Plan on a client’s device.

  • Navigate to GIRDER.BEAM.CARE
  • Log in with your Girder credentials.
  • Select the system of the person you support.
  • Press Configure Plan in the dark menu area.
  • Scroll to the bottom of the Configure Plan page and press the Flourish Versions button
  • You can press each entry of this page to view and edit current plans, or view or restore previous plans. Next time the Flourish user starts the app, they will be notified of new data.

Creating printable Flourish plans

Press Generate Flourish PDF in the menu area of GIRDER.BEAM.CARE

A download will automatically start, or you browser may open the PDF at which point you can save it to your device or share it with others.
